Based on observation, interview and record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPA observed cleaning products in the cabinet under the bathroom sink that are accessible to children in care. Licensee stated that the children use the kitchen area to wash hands and LPA observed disinfecting wipes, glass cleaner, diswasher tablets and other cleaning products in cabinets that are accessible to children in care.
Resolution: Licensee to submit a written plan of correction to LPA by 07/25/2025.
Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. The smoke detector in the daycare/family room was not observed to be functional. No other functional smoke detectors were observed. Licensee stated that she has purchased another and will installl it.
Resolution: Licensee to submit proof of functional smoke detector to LPA by 08/22/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above for staff, S1 and S2 which poses a potential health, safety or personal rights risk to persons in care. Both assistants were observed to be missing the required Mandated Reporter Training.
Resolution: Licensee to have both staff, S1 and S2 complete the Mandated Reporter Training and submit certificate of completed training to LPA by 08/22/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above for both staff, S1 and S2 which poses a potential health, safety or personal rights risk to persons in care. Upon arrival staff, S1 was observed alone with children as Licensee was out for an appointment. LPA reminded Licensee that atleast one staff caring for children is required to have current Pediatric CPR/ First Aid Training.
Resolution: Licensee to have staff, S1 complete the EMSA certified Pediatric CPR/ First Aid Training and submit proof of completed training to LPA by 08/22/2025.
Based on observation, interview nad record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA did not observe required Notification of Parents' Rights (PUB 394), Earthquake Preparedness (LIC 9148) and Emergency Disaster Plan (LIC 610A) posted at the facility.
Resolution: Licensee to post missing forms and submit picture proof to LPA by 08/22/2025.

Based on observation, interview and record review the licensee did not comply with the section cited above for two infants, C6 and C7 which poses a potential health, safety or personal rights risk to persons in care. Upon review of sleep log LPA observed that 15 minute sleep checks were not conducted for one infant (C6) since June and for another (C7) since May.
Resolution: Licensee to conduct 15 minute sleep checks for both infants, C6 and C7 and submit proof of completed checks to LPA by 08/22/2025.
Based on observation and interview, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. During facility walk through, LPA observed an odor in the children's bathroom and cat food on the floor in the kitchen area which is accessible to children. In the outdoor area, LPA did not observe sufficient tan bark under swings and observed the weed liner torn and bunched under the play structure and swings. LPA observed several rods and miscellaneous items including a leaf blower in the outdoor area that is accessible to children.
Resolution: Licensee to submit a written plan of correction outlining the measures the facility will take to provide safe and healthful environment for children in care. Plan of correction due date is 08/22/2025.
Based on observation, the licensee did not comply with the section cited, which poses an immediate health, safety or personal rights risk to persons in care. LPA observed that there were knives in the kitchen drawer, which is accessible to children.
Resolution: Licensee moved knives to the corner where it is inaccesible to children during today's inspection.

Resolution: By POC 10/07/2022, Licensee will submit an updated LIC 999A to reflect the change to the on-limit area.
