Based on LPAs observations C1 and C3 were swaddled during nap time. The licensee did not comply with the section cited above in 2 out of 4 infants which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a written statement to LPA Raja, stating that she understands the safe sleep regulations and ensure that she does not swaddle infants at all times. Licensee will submit written statement to LPA by POC date of 03/08/2023. AB 633 requirements was explained to licensee and she understood them.
Based on observation, interview and record review, the licensee did not comply with the section cited above in C1, C2 and C3 missing LIC 9227 Infant Sleeping Plan which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a copy of LIC 9227 for all 3 infants to LPA Raja by POC date 03/21/2023. Licensee was able to have authorized representatives complete LIC 9227 for infants C1, C2 and C3 and showed proof to LPA on day of inspection (03/07/2023).
