Based on observation and interview, the licensee did not comply with the section cited above. LPA observed licensee added a day care room to the home and did not notify the Department which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit and updated facility sketch to CCLD by POC date. Licensee was informed a new fire clearance will be required.

Based on interview, the licensee did not comply with the section cited above. Licensee stated she is in process of obtaining a permit for addition to the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof she is in process of obtaining a building permit and understands room cannot be used for day care use until the room is approved and inspected to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed two infants asleep with blankets and/or pillows in their play yards which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee and her assistants will review Safe Sleep Regulations and submit a statement stating they have read an understand regulations to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Assistant Jennifer Garcia has not completed Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Mandated Reporter Certificate for Jennifer Garcia to CCLD by POC date. Training can be found at www.mandatedreporterca.com.

Based on record review, the licensee did not comply with the section cited above. Child 4, 5, and 7 are missing LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC9227 for child 4, 5, and 7 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above in having form LIC9227 in one infant file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete form LIC9227 for one infant in care and will submit a copy to Licensing Department by 11/10/21
