Based on observation, the licensee did not comply with the section cited above as two play yards had blankets inside which poses a potential health, safety or personal rights risk to persons in care.

Resolution: During today's inspection licensee removed the blankets and placed on the top of a shelf. LPA and Licensee discussed the safe sleep regulation and Licensee understands that blankets shall not be used by infants when they sleep in the play yard.

Based on record review, LPA observed that the licensee and one adult assistant's mandated reporter training was expired which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and one adult assistant will complete mandated reporter training. Licensee will provide a statement of how she will remember to complete this every two years. Licensee with submit these two items to the department on POC due date 02/17/2026.

Based on record review, the licensee did not comply with the section cited above as one adult assistant was missing immunization records for flu, pertussis and measles which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide the immunization record for pertussis, measles and flu for one adult assistant to the department by POC due date 02/17/2026.
