Based on interview, the licensee did not comply with the section cited above. Licensee stated she does not have a Safe Sleep log for infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Safe Sleep log for all infants in care beginning 05/03/2024 through 05/10/2024 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed during nap time two infants sleeping in separate play yards with blankets which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating her and her assistants have read Safe Sleep regulations to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed one infant sleeping with a pacifier and chord attached to it which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed a bouncer/rocker in the home not being used by children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed bouncer and understands such items are not allowed on the premises. Deficiency cleared today.

Based on record review, the licensee did not comply with the section cited above. Daughter Cristel does not have a Mandated Reporter certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a current Mandated Reporter certificate for daughter Cristel to CCLD by POC date.

Based on observation and interview, the licensee did not comply with the section cited above. LPA observed a bathroom being built in the garage that licensee failed to notify the Department of the change which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA obtained a permit for additional bathroom during visit. Licensee will submit a new sketch and notify the Department when bathroom is completed. LPA notified licensee a new Fire Inspection will be required.

Based on record review, the licensee did not comply with the section cited above. Child 9 is missing LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC9227 for child 9 to CCLD by POC date.

Based on observation, interview and record review, the licensee did not comply with the section cited above in assistant Silvia, CPR expired on 8/26/2022, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of current CPR card for assistant to the San Jose Regional Office by close of business 01/16/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above in Room 3 was converted for child care use, room 1 is now off limits, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Applicant will submitt a written plan stating she has reviewed and understand the requirements to notify the department by close by business 01/09/2023.

Based on observation and record review, the licensee did not comply with the section cited above in child 3 (c3) and c4 missing proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of immunization cards for C3 and C4 by close of business to the San Jose Regional Office 01/16/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above, licensee Rosa Ruiz completed on 10/20/2020 and assistant Silvia Ruiz completed on 4/29/2020, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of completion of Mandated Reporter Training to the San Jose Regional Office by close of business 01/16/2023.
