Based on record review, the licensee did not comply with the section cited above in 2 out of 2 infants (C1&C3), which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By plan of correction due date, 01/29/2025, Licensee will submit the copy of the individual infant Sleeping plan (LIC9227) to the department for infants(C1&C3).
Based on interviews and record review,, the licensee did not comply with the section cited above for infants (C1& C3) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By plan of correction due date, 01/29/2025, Licensee will submit the copy of the infant safe sleep log to the department for infants(C1&C3).
Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee did not obtain a criminal record clearance for an adult renter who started to live in her home on 4/15/22, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The Licensee submitted a written statement to the Department on 04/28/22 indicating that she understands that all adult(s) must obtain the required criminal record and child abuse index clearances prior to working or residing in the home. Licensee will submit criminal record fingerprint clearance for adult renter by POC date. Civil penalty of $500 assessed today (see LIC 421BG). The Licensee must provide copies of this report to parents/guardians of children in care at this facility and to parents/guardians of children newly enrolled at this facility during the next 12 months per the AB633 requirements.
Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee's daughter/adult assistant in child care, does not have a current Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit a current Mandated Reporter training certificate for daughter/adult assistant by POC date.
