Personnel Records (a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee.... This requirement has not been met as evidenced by: LPA observed 2 out of 4 staff files missing immunization records and/ or physician's report and TB test or assessment. This poses a potential risk to the health, safety and personal rights of children in care.

Based on observation, children went outside at 11AM. Their water bottles were brought outside at 11:09AM, which poses a potential health and safety risk to children in care.

Based on observation, LPA observed that one staff was with 14 children at 9:43AM and 10:24AM. LPA observed at 10:31AM that staff left 14 children with therapist. At 11:54AM, LPA observed that there was one staff with 16 children. This poses an immediate health and safety risk to children in care.

Based on interview, the school-age children and preschool children commingling. This poses an immediate health and safety risk to children in care.

Based on observation, LPA observed at 10:01AM and 10:46AM that there were children in the restroom. There was no staff positioned where they can visually supervise the children. LPA also observed at 3:05PM that there was a child in Room 2. The staff was standing by the hallway that leads to the restroom. This poses an immediate health and safety risk to children.

Based on observation, LPA observed that a staff grabbed a child by the wrist and had them sit down on a chair. LPA also observed that a child told to sit down for 10:29AM till 10:37AM. This poses an immediate health and safety risk to children in care.

Child's Record (a) A separate, complete and current record for each child is maintained in the child care center. This requirement has not been met as evidenced by: 3 out of 4 children's files reviewed were missing immunization records and a physician's report with TB test or assessment. This poses a potential threat to the health, safety and personal rights of children in care.

(c)(1) A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation. This requirement has not been met as evidenced by: 4 out of 4 children's files were missing 2 or more LIC9224 receipts documenting Type A Citations or an Office visit dated: 07/15/2025, 04/15/2025, 10/17/2024, 08/07/2024, 07/25/2024. This poses a potential risk to the health, safety and personal rights of children in care.

Indoor Activity Space for School-Age Children (b) In combination programs, indoor activity space provided for school-age child care center children shall be physically separated from space provided for infant care and child care center children. This requirement has not been met as evidenced by: :30 AM LPA observed Director Dinora Sanchez in one classroom with the school age children and preschool children commingled together for drop off until 8:00 AM where there was a total of 10 children in one classroom (5 preschool age and 5 school age). This poses a potential risk to the personal rights, health and safety of children in care.

Based on observation, there is a metal cabinet that is outside of Room 1 that has mouse dropping inside and on the floor to the right of cabinet. This poses a potential health and safety risk to children in care.

Based on observation, there were plastic bins in Room 1 that was cracked and used for toys. LPA also observed that the wooden play structure on the left hand side was missing a wooden plank on the top causing there to be a gap in the floor. There is also a plastic play structure that is cracked. Area that is cracked is sharp. LPA also observed that the tarp outside is coming up, which could cause a tripping hazard. This poses a potential health and safety risk to children in care.
