Resolution: Licensee states that she will complete an In-Person Pediatric CPR/First Aid and will submit proof of completion by 01/08/26.

Based on record review, the licensee did not comply with the section cited above in 1 out 3 children in care which poses a potential health, safety or personal rights risk to persons in care. C3 is missing proof of immunizations.

Resolution: Licensee states that she will request obtain a copy of C3's immunization record and will submit completed California School Immunization Record by 12/16/25.

Based on record review, the licensee did not comply with the section cited above. Licensee does not maintain sleep/nap log for child (C3) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that she will complete a week sleep log for C3 and will submit documentation by 12/16/25.

Per LPA's review of files, Licensee does not have Mandated Reporter Training per AB1207. Licensee stated she was unaware of the requirement. This poses a potential risk to the health and safety of children in care.

Resolution: BY POC DUE DATE Licensee will complete the training and send a copy of the Certificate of Completion to Licensing Office. LPA provided the web address https://www.mandatedreporterca.com to access the required training.
