Based on record review and interview, the licensee did not comply with the section cited above for herself and assistant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and assistant will complete the Mandated Reporter Training and submit proof to LPA by plan of correction date of 04/19/2024. LPA reminded Licensee that this training needs to be renewed every two years.

Based on record review and interview, the licensee did not comply with the section cited above for herself and assistant which poses a potential health, safety or personal rights risk to persons in care. LPA did not observe CPR/First Aid certification for either staff.

Resolution: Director or assistant will complete the Pediatric CPR/First Aid Training and submit proof to LPA by plan of correction date of 04/19/2024. LPA reminded Licensee that this training needs to be renewed every two years.

Based on record review and interview, the licensee did not comply with the section cited for three children (C6-C8) in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain missing immunizations for the three children (C6-C8) and submit proof to LPA by plan of correction date of 04/19/2024.

Based on record review and interview, the licensee did not comply with the section cited above for four children (C1, C6-C8) in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain missing Affidavit Regarding Liability Insurance for Family Child Care Home (LIC 282) for the four children (C1, C6-C8) and submit proof to LPA by plan of correction date of 04/19/2024.
