Based on interview and record review, the licensee did not comply with the section cited above in 2 infants which poses a potential health, safety or personal rights risk to persons in care. Licensee stated that she thought the 15 minute sleep checks need to be conducted for children only until they are one year old. LPA informed Licensee of the requirement to conduct the checks until infants are 2 years of age.
Resolution: Licensee to conduct 15 minute checks for two infants in care and submit proof to LPA along with a plan of correction by 11/28/2025.
Based on record review, the licensee did not comply with the section cited above. The licensee’s adult son residing in the home did not have background clearance, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: The licensee will have her son finish the finger print by the due day and send LPA a copy of proof for the finger print.
