Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed an infant napping in a play yard with a pacifier that had a clip holder attached to it.

Resolution: Licensee to review safe sleep regulations and submit a written plan of correction to LPA by 02/06/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During file review, LPA observed that assistant (S2) is missing the Mandated Reporter Training.

Resolution: Licensee to have staff (S2) complete the Mandated Reporter Training and submit completion certificate to LPA by 02/06/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During file review, LPA observed that assistant (S2) is missing proof of tuberculosis test and immunizations for measles, pertussis and influenza/statement declining influenza.

Resolution: Licensee to submit proof of tuberculosis test and immunizations for staff S2 to LPA by 02/06/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During file review LPA observed that three children (C3, C4 and C5) are missing the Consent for Emergency Medical Treatment (LIC 627) and signature on other forms.

Resolution: Licensee to have parents/authorized representatives complete forms and submit proof to LPA by 02/06/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Upon review of the facility roster LPA observed that it was not current.

Resolution: Licensee to submit a current roster and plan of correction to LPA by 02/06/2025.

Based on observation and interview, the licensee did not comply with the section cited above for one infant, C1 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to conduct and document 15 minute nap checks and submit a copy of documented nap check to LPA by plan of correction date of 04/30/2024.

Based on record review, the licensee does not have an Emergency Diaster Plan (LIC 610 A) for review at facility which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a completed Emergency Diaster Plan (LIC 610A) by Plan of Correction date of 15 March, 2023.

Based on record review, the licensee and spouse who works with children do not have a current mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a completed Mandated Reporter Training Certificate for licensee and spouse by Plan of Correction date of 3 April, 2023.
