Licensee did not have proof of immunizations for MMR (measles) and no proof of immunizations for Tdap, MMR and Influenza were observed for assistant.
Resolution: Licensee will submit required proof of immunizations for herself and staff to LPA by Plan of Correction date of 03/08/2024.
