Based on record review, the licensee did not comply with the section cited above. Child 5 and 8 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed child during visit. Licensee will submit a written statement stating she understands carseats are only used for cars and not for sleeping.

Based on LPA's observation, the licensee did not comply with the section cited above. LPA observed two children being put to sleep in highchairs. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed children during visit. Licensee wil submit a written statement stating she understands children should receive comfortable accomodaations when sleeping. AB633 Parent Notification is required. This page shall be provided to all parents of children currently enrolled and any future children being enrolled for the next 12 months per AB633 requirements.
