Based on observation, interview and record review, the licensee did not comply with the section cited for herself which poses a potential health, safety or personal rights risk to persons in care. Licensee has not renewed the Mandated Reporter Training.

Resolution: Licensee to complete training and submit certificate of completion to LPA by plan of correction date of 10/07/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above for two children (C2 and C3) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to obtain missing immunization records for two children and submit to LPA by due date of 10/07/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above for four children (C1, C2, C3 and C5) which poses a potential health, safety or personal rights risk to persons in care. Licensee does not have liability insurance for the day care and was missing Affidavit Regarding Liability Insurance for Family Child Care Home (LIC 282) for four children.

Resolution: Licensee to obtain missing form from parents and submit to LPA by due date of 10/07/2025.

Based on interview and record, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have a currect Fire Drill log.

Resolution: Licensee will conduct and document a fire drill and submit proof to LPA by Plan of Correction date of 09/20/2023.

Based on interview and record review, the licensee did not comply with the section cited above for 1 out of 3 staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of immunizations for MMR (measles ), Tdap (Pertussis) and Influenza/ statement declining influenza for staff, S2, by Plan of Correction date of 09/20/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have a current facility roster.

Resolution: Licensee will submit a current facility roster to LPA by Plan of Correction dat of 09/20/2023.
