Based on observations, the licensee did not comply with the section cited above in 3 out of 3 cribs had foam attached with zip ties, towels draping over the crib or items stored inside a crib used for infant sleep. Children were not sleeping in the cribs at the time of observation. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Remove all items attached to the crib. Determine alternative location to store blankets, stuff animals and sheets to ensure the crib is not contaminated by the sharing of other children in care's possessions, blankets or toys.
Based on observations, the licensee did not comply with the section cited above. Fire extinguisher was manufactured in 2021 and had not been serviced since purchase, this poses a potential health, safety or personal rights risk to persons in care.
Resolution: Replace or service fire extinguisher Annually in accordance with State Fire Marshall Regulations.
Based on record review, the licensee did not comply with the section cited above in 7 out of 13 files were missing immunization records or the records had not been updated in over one year. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to collect missing immunization records, update all children's records as necessary and provide the Department with a written plan on how to manage immunizations so that all children in care have up to date records.
Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed there were blankets, pillow and toys in the crib and one infant was sleeping there.
Resolution: Licensee will ensure that the crib or play yard is free from all loose articles and objects. Licensee will remove the blankets, pillows, and toys from the crib,
Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. At time of arrival at 8:35 AM the staff did not granted access to Licensing Program Analysts (LPAs) in the day care.
Resolution: By the Plan of correction due date 05/22/2025, Licensee will submit the written statement of understanding of Title 22 regulations; inspection authority of the department and the staff is aware of the inspection authority of the department, to the department.

Licensee did not provide the LIcensening Report to the parents of currently and newly enrolled children. Licensee did not obtained the LIC 9224 for Typa A cited on 11/06/2024.
Resolution: By the Plan of Correction (POC) due date, 05/22/2024, Licensee will obtain the signed copy of LIC 9224 for all currently and newly enrolled children in the day care. Licensee will provide signed copies of the form LIC 9224 to the department as a proof of correction.
Based on Child's record review for C1 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By POC due date of 11/12/2024, Licensee will provide copy of 15 minute sleep log for C1.
Based on observation of (14) children (1 infant and 13 preschool age) in the home during today's inspection, Licensee did not have any school age children present which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: By POC due date of 11/07/2024, Licensee will submit a written statement of "understanding" the requirements for the maximum number of children for whom care may be provided for more than twelve and up to fourteen children only if the criteria in Section 1597.465 of the Health and Safety Code are met.
