Based on observation and interview, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Upon arrival, LPA observed Licensee with 11 preschool children without an assistant. LPA provided Licensee with a copy of the ratio worksheet for reference.

Resolution: Licensee to submit a written plan of correction to LPA by 01/31/2026.

Based on observation, interview and records review, the licensee did not comply with the section cited above for herself and assistant which poses a potential health, safety or personal rights risk to persons in care. Licensee and assistant have not completed the required Mandated Reporter Training for Child Care Workers. Licensee's training expired on 09/21/2025 and assistant does not have previous training.

Resolution: Licensee and assistant to submit certificate of completed training to LPA by 02/20/2026.

Based on observation, interview and records review, the licensee did not comply with the section cited above for assistant, S2 which poses a potential health, safety or personal rights risk to persons in care. Assistant is missing proof of measles and influenza immunization's along with Employee Rights and Statement Acknowledging Requirement to Report Child Abuse forms.

Resolution: Licensee to submit the missing records and a written plan of correction to LPA by 02/20/2026.

Based on observation, interview and records review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did not have an updated Facility Roster will all the information required for children.

Resolution: Licensee to submit an updated children's roster and a written plan of correction to LPA by 02/20/2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above for one child (C6) which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to submit missing form to LPA by plan of correction date of 10/30/3024.

Based on interview and record review, the licensee did not comply with the section cited above for 3 out of 3 staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete immunizations for MMR, Tdap and Influenza and submit proof to LPA by Plan of Correction date of 09/20/2023.

Based on observation, interview and record review, the licensee did not comply with the section cited above for 3 out of 3 staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of Mandated Reporter Training for herself and two staff to LPA by Plan of Correction date of 09/202/2023.
