Based on observation and interview, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Upon arrival LPA observed assistant (S1) with ten preschool children who were observed to be waking up from nap. Assistant stated that Licensee is out for an appointment and will return soon. Licensee arrived in 10 minutes. LPA provided Licensee with a copy of the ratio worksheet for reference.
Resolution: Licensee to submit a written plan of correction to LPA by 02/12/2026. Licenseee submitted a hand written plan of correction to LPA at the end of today's inspection.
Based on observation and record review, the licensee last conducted the fire drill in April, 2022 and did not comply with the section cited above which poses a potential health, safety or personal righ ts risk to persons in care.
Resolution: Licensee shall conduct a fire drill with the children in care, and document the drill on the fire drill log. The licensee will submit a copy of the fire drill lot to LPA by the Plan of correction date of 04/13/2023.
Based on observation and record review, the licensee did not comply with the section cited for herself and staff which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete the training online and submit proof of completion for her and one staff by Plan of Correction date of 04/13/2023.
