Based on observation, the licensee did not comply with the section cited above in two out of three cribs had rail guard attached to the tops of the cribs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed both rail guards during the time of inspection. LPA and licensee reviewed the infant sleep regulation and a copy was left at the facility.
Based on record review, the licensee did not comply with the section cited above due to infant file not having documentation of 15-minute sleep log which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to start documenting the 15-minute sleep logs from 11/21/25-12/03/25 and submit to licensing by plan of correction due date: 12/04/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee did not renew her Pediatric First aid and cardiopulmonary resuscitation (CPR) training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to LPA a current Pediatric and CPR training certificate by POC date 06/24/22.
Based on observation, interview and record reviews, the licensee did not comply with the section cited above. Licensee did not renew her Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to LPA a current Mandated Reporter Training Certificate by POC date 06/24/22.
