Based on record review, the licensee did not comply with the section cited above in which poses potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date of 7/18/2025, Licensee will provide proof of Staff S(1) Measles, mumps and rubella immunization.

Based on record review, the licensee did not comply with the section cited above in which poses potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date of 7/18/2025, Licensee will provide proof of daily infant 15-min check sleep log.

Based on observation, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Resolution: By POC due date of 7/18/2025, Licensee will provide Proof of: Pool alarm that meets ASTM F2208 or Pool cover that meets ASTM F1346-23, Life Ring exterior diameter 17 inches that meets United States Coast Guard and daily inspection of the drowning prevention safety features and safety.

Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee did not maintain a document of fire drill conducted within the last six months which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit to LPA a proof of document that fire drill was conducted and completed by POC due date. Licensee will submit a statement of understanding of the regulation.

Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee did not maintain every 15-minute sleep checks on infant's files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit to LPA by POC due date documentation of each enrolled infant's 15-minute sleep check records. Licensee will also submit statement of understanding of safe sleep regulations.

Based on observation, interview and record review, the licensee did not comply with the section cited above. Licensee has made current alterations in the home of patio conversion to sun room without notifiying the Department which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a plan of correction to ensure understanding of regulation requirement. Licensee will submit to LPA a complete, updated facility sketch of indoor and outdoor areas of the home by POC due date. Licensee will not use sun room for day care purposes without passing fire clearance inspection by Local Fire Department.
