Based on record review, the licensee did not comply with the section cited above. Licensee's helper (S2) is missing immunizaitons in measles and pertussis which pose a potential health, safety or personal rights risk to persons in care.

Resolution: Kimeta states that she will submit proof of immunizations in measles and pertussis for S2 by 09/26/25.

Based on observation, the 3A40BC fire extinguisher is empty. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Kimeta states she will purchase a required size fire extinguisher and will submit proof of purchase and photo showing that it's mounted between 3 to 5 feet above the floor.

Based on record review, five children C-1 thru C-5 are missing the Consent for Emergency Medical Treatment (LIC 627) and three children C-4 thru C-6 are missing the Identification and Emergency Information (LIC 700) and one child (C-3) is missing the second page of the LIC 700. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Kimeta stated that she will submit completed forms and signed forms by 07/11/24.

Based on record review, the licensee did not comply with the section cited above in 6 out of 6 persons which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Kimeta states she will submit completed Affidavit Regarding Liability Insurance form for the six children by 07/11/24.

Based on record review, C-4 is missing LIC 9227 in the file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Kimeta states she will send completed LIC 9227 for C-4 by 07/11/24.

Based on record review, Licensees, Kimeta and Adzija, and assistant (S-1) have expired Mandated Reporter Training., This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Kimeta stated that she will submit proof of current Mandated Reporter Training for herself, Adzija, and her assistant by 07/11/24.

Based on record review, both licensees and licensee's helper (H1) do not have proof that they completed the Mandated Reporter Training. This poses a potential risk to the health, safety or personal rights of children in care.

Resolution: Kimeta states that she will submit proof that herself, her husband, and her helper completed the Mandated Reporter Training by 07/28/22.

Based on record review, licensee does not maintain a current children's roster. This poses a potential risk to the health, safety or personal rights of children in care.

Resolution: Kimeta submitted a copy of current children's roster during the inspection. Deficiency corrected.

Based on LPA's observation, licensee's helper was alone in the home supervising seven children including four infants and three preschool age. This poses an immediate risk to the health, safety or personal rights of children in care. Upon receipt, licensee shall post and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months.

Resolution: Licensee states she will submit a written to ensure that she maintains the required ratio and capacity at all times. Licensee submitted a written plan during the inspection.
