Based on interview, the licensee did not comply with the section cited above. Licensee stated she does not have a safe sleep log for infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of safe sleep log for week of 11/08/2021-11/10/2021 to LPA by 11/19/2021.

Based on observation, the licensee did not comply with the section cited above. LPA observed animal feces in the back patio used for outdoor play which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will clean backyard and submit photos to LPA by 11/06/2021. AB633 Parent Notification is required. This page shall be provided to all parents of children currently enrolled and any future children being enrolled for the next 12 months per AB633 requirements.

Based on record review, the licensee did not comply with the section cited above. LPA reviewed children's sign in sheets and observed on October 1, 4-9, 11-15 and 18-22, Licensee was out of ratio. Licensee had three infants, four preschools and one child in Kindergarten which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating how she plans on remaining in ratio to LPA by 11/19/2021.
