Log shows that the last Fire Drill was conducted on 03/21/2024 which is not compliant with the six-month requirement for facilities. LPA reminded Owner that fire drills needs to be conducted every 6 months and documented.

Based on interviews, staff stated that an unusual incident occurred on 05/09/2024 involving two staff (S1 and S2) in which they were arguing in front of children in care. This poses a potential risk to the health, safety, and personal rights to children in care.
Based on observation, interview and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Based on interview with Director and staff, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. On the day of the incident (05/09/2024) staff, S2 left staff, S1 in the room alone with 19 children.
