Based on record review and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee has not renewed the Mandated Reporter Training which expired on 08/10/2025. Resolution: Licensee to submit proof of completed training to LPA by plan of correction due date of 03/27/2026.

Based on record review and interview, the licensee did not comply with the section cited above for one child which poses a potential health, safety or personal rights risk to persons in care. Child was observed to be missing proof of updated immunization's required for their age, consent for emergency medical treatment (LIC 627) and Notification Parent's Rights (LIC 995A) forms.

Resolution: Licensee to submit missing documents to LPA by plan of correction due date of 03/27/2026.

Based on observation and record review, the licensee did not comply with the section cited above 3 out of 3 staff which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and two staff will complete the Mandated Reporter Training online and submit proof of completion to LPA by Plan of Correction date of 08/23/2023.

Based on record review, the licensee did not comply with the section cited above for one infant in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct and document 15-minute sleep checks for one infant and submit proof of checks to LPA by Plan of Correction date of 08/23/2023.
