Based on observation, interview and record review the licensee did not comply with the section cited above for one infant (C1) which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee to have parent/authorized representative complete the missing form and submit it to LPA by due date of 07/31/2025.
Based on observation, interview and record review, LPA observed one infant (C2) asleep in a pack and play with a folded blanket and milk bottle inside the crib. LPA observed another infant's pack and play, where infant was not napping, but the pack and play had attachments on the side of the crib. This poses a potential health, safety or personal rights risk to persons in care. Licensee removed all items during today's insepction.
Resolution: Licensee to submit a written plan of correction to LPA by due date of 07/31/2025.
