During today's inspection, LPAs observed 37 children with 2 staff. The facility is operating out of ratio requirements which poses an immediate health, safety or personal rights risk to persons in care. Substitute Director stated that staff called in sick and another staff is on the way.

Based on file review, LPA observed that staff, S2 is missing proof of negative tuberculosis test. This poses a potential risk to the health, safety and personal rights of children in care.

Based on interview and record review, the director did not comply with the section cited above as the facility roster has not been updated, which poses a potential health, safety or personal rights risk to persons in care.

During today's inspection, LPAs did not observe the Facility Inspection Report including the LIC 809, LIC 809C and LIC 809D pages with the Type A citation issued on 11/07/2024 posted at the facility. This report was required to be posted for 30 days.

LPA observed 41 children and 2 staff during today's inspection. Director stated that they are short staffed and working to hire new staff.

(c)(1) A licensed child day care facility shall provide to the parents or guardians of each child receiving services in the facility copies of any licensing report that documents any Type A citation that represents an immediate risk to the health, safety, or personal rights of children in care…(4) The licensee shall keep verification of receipt in each child's file.

(2) Each person specified in (g) above shall have a health-screening report signed by the person performing the screening. This report shall indicate the following:
