Based on observation, interview and record review, the licensee did not comply with the section cited above for staff S2 and S3 which poses a potential health, safety or personal rights risk to persons in care.

Based on interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Facility has not completed water lead testing by the deadline.

Personal Rights (a) The licensee shall ensure that each child is accorded the following personal rights:(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning. This requirement is not met as evidence by: Based on information gathered on 4/30/2025 video footage show staff (S-1) was redirecting child (C-1) during in outdoor incident were staff (S-1) mishandled child (C-1) resulting in bruising and scratches. This poses an immediate risk to the health, safety, and personal rights of children in care.
