Based on observation, the licensee did not comply with the section cited above. LPA observed an infant swaddled sleeping in a play yard. LPA observed a barricaded fireplace in the home which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the blanket during the visit and stated she understands she may not swaddle infants.
Based on record review, the licensee did not comply with the section cited above. LPA did not observe a completed Safe Sleep log for infant in care which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit past Safe Sleep logs for the month of February and March for child 7 to CCLD by POC date.

Based on observation and record review, the licensee did not comply with the section cited above, assistant Guadalupe Gonzalez and Evelia Ornelas did not have TB clearance on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of TB clearance for Guadalupe and Evelia to the San Jose Regional Office by close business 10/04/2022.
