Based on record review, the licensee did not comply with the section cited above. Child 3, 4, 5 and 7 immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit updated immunization records for child 3, 4, 5 and 7 to CCLD by POC date.

Based on observation and record review, the licensee did not comply with the section cited above. LPA observed two infants sleeping on their tummies and do not have a Safe Sleep plan on file which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee and her assistants will Safe Sleep regulations and licensee will submit a statement stating all have read and understand regulations to CCLD by POC date. LPA provided a copy of Safe Sleep Regulations to licensee.

Based on observation and record review, the licensee did not comply with the section cited above. The facility was caring for four preschool children and five infants. Licensee was out of ratio which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating how she will remain in ratio to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Licensee did not have a fire and disaster drill log. Licensee stated she was not aware she needed to practice drills which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and disaster drill and submit log to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. One infant had a blanket in the play yard in lieu of a fitted sheet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed blanket during visit and understands she may only use a fitted sheet for play yards. Deficiency cleared during visit.

Based on record review, the licensee did not comply with the section cited above. LPA observed licensee assistant Maria does not have immunization records on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunization records for assistant Maria to CCLD by POC date.

Based on interview and record review, the licensee did not comply with the section cited above. Child 2, 11 and 12 are missing LIC995 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC995 for child 2, 11 and 12 to CCLD by POC date.

Based on interview and record review, the licensee did not comply with the section cited above. Two children (11 and 12) do not have files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC282 for child 11 and 12 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 8, 9, 10 and 11 are missing LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC9227 for child 8, 9, 10 and 11 to CCLD by POC date.

Based on interview and record review, the licensee did not comply with the section cited above. Licensee did not have a Safe Sleep log for infants sleeping during visit which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Safe Sleep log for all infants in care for week of 01/29/2024 - 02/02/2024 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. LPA did not observe a current roster of the children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit an updated roster to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Two children (11 and 12) do not have files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit files for child 11 and 12 to CCLD by POC date.

Based on interview and record review, the licensee did not comply with the section cited above. Child 7, 11 and 12 are missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC627 for child 7, 11 and 12 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed several razors and Clorox wipes accessible to children in bathroom used by children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed items during visit. Deficiency cleared during visit.

Based on observation and record review the licensee did not comply with the section cited above. Licensee was alone with 4 infants and 3 pre school aged children while her assistant was on lunch which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a written plan of correction stating what she will do to prevent this from reoccurring.

Based on observation and record review interview, the licensee did not comply with the section cited above; Licensee did not check on children napping in bedrooms with closed doors this poses a potential health, safety or personal rights risk to persons in care.

Resolution: If an infant sleeping in a seprate room from where the provider is stationed, the door to the room from where the provider is stationed must be open. Licensee is to read section 102425 Infant Safe Sleep and submit a written statement stating she understands the regulation.

Based on record review the licensee did not comply with the section cited above. Infants are misisng the Individual Sleeping Plan (LIC 9227) in their file this is a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit a copy of LIC 9227 for infants in care and keep original in infants file. Copy of LIC 9227 to be emailed or mailed to LPA elizabeth.berumen@dss.ca.gov

Based on observation and record review The Licensee has not began to utilize the LIC9227 form. This poses a potential risk to the Health, Safety or Personal Rights to children in care.

Resolution: The LPA provided the Licensee with the LIC9227 form. The Licensee will immediately begin to utilize the form and ensure it is present in all the infants files.

Based on LPA observation and interview, the licensee did not comply with the section cited above. Infant was asleep in off limit bedroom on a couch with the door closed. Licensee states infant won't nap with others and will wake up others so she naps infant in that room which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a written plan of correction stating she understands safe sleep practices and that she understands she can not force an infant to sleep, infants are to sleep in crib or playpen.
