Resolution: Licensee will submit a updated Immunization records for child 2 to CCLD by POC date

Based on interview, the licensee did not comply with the section cited above. LPAs observed that Licensee did not have an infant safe sleep log for Child number 2 in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a Safe sleep Log for Child 2 starting 4/11/2025 through 4/25/2025 to CCLD By POC date.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in LIC279 Application for Child Care Home is not updated with current adult living in licensee child care home, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit updated LIC 279 to the San Jose Regional Office by close of business 04/28/2023.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in licensee does not have assistant Sylvia and Yoselyn proof of immunization and TB testing which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: licensee will submit copies of proof of vaccinations for both assistants to the San Jose Regional Office by close of business 05/26/2023.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in, two adult resident daughters Yoselyn and Lizbeth do not criminal background clearance, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of background clearance to the San Jose Regional Office and adult daughters will not be present until criminal background clearance is obtained.

Resolution: Licensee will submit a copy of completed Mandatory Reporter Training to the San Jose Regional Office by close of business 05/12/2023.
