Based on observations during facility walk through, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed cleaning products and razors in the cabinet under the bathroom sink. LPAs also observed medications on the kitchen counter. LPAs reminded Licensee that all shampoos, soap, medication, mouthwash, perfumes, razors, cleaning products, air fresheners, and nail polish/remover are to be placed inaccessible to the children or in child locked cabinets.

Resolution: Licensee to submit picture proof and a written plan of correction to LPAs by 05/31/2025.

Based on observation and interview, the licensee did not comply with the section cited above for one infant (C1) which poses a potential health, safety or personal rights risk to persons in care. LPAs observed child in a play yard covered with a blanket.

Resolution: Licensee to review safe sleep regulations and submit a written plan to LPA by 05/31/2025.

Based on interview and record review, the licensee did not comply with the section cited above for herself which poses a potential health, safety or personal rights risk to persons in care. Licensee has not renewed her mandated reporter training.

Resolution: Licensee to complete training and submit certification of completion to LPA by plan of correction date of 06/13/2025.

Based on record review and interview, the licensee did not comply with the section cited above for assistant, S2 which poses a potential health, safety or personal rights risk to persons in care. S2 was observed alone with children but has not completed the Pediatric CPR/First Aid training.

Resolution: Licensee to have assistant complete the training and submit certificate of completion to LPA by plan of correction date of 06/13/2025.

Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed day care children napping in master bedroom which is off limits to children.

Resolution: Licensee stated that she will discontinue to use the off limit area. Licensee will submit a written plan of correction stating she understands that children are not to be in off limit areas, and she will contact the department prior to any changes. POC due date 06/13/2025.

Based on record review and interview, the licensee did not comply with the section cited above for assistant, S2 which poses a potential health, safety or personal rights risk to persons in care. Assistant does not have the required immunization's in file for measles, pertussis and influenza/ statement declining influenza.

Resolution: Licensee to obtain proof of immunizations for S2 and submit to LPA by plan of correction date of 06/13/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPAs observed that Licensee has not maintained a file with the required forms for child C3. Child C5 is missing their immunization record and children, C4 and C6 are missing the Parent's Right's form (LIC 995).

Resolution: Licensee to obtain all missing items and submit to LPA by plan of correction date of 06/13/2025.

Based on record review and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee has not maintained an updated roster.

Resolution: Licensee to update roster and submit to LPA by plan of correction date of 06/13/2025.

Based on observation, interview and record review, the licensee did not comply with the section cited above for three infants (C1, C2 and C3) which poses a potential health, safety or personal rights risk to persons in care. Licensee has not maintained a 15-minute nap log for all infants in care.

Resolution: Licensee to conduct and documents 15 minute nap checks and submit proof to LPA by plan of correction due date of 06/13/2025.
