Based on interview, the licensee did not comply with the section cited above. Licensee did not have completed Mandated Reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a current Mandated Reporter certificate to CCLD by POC date. Training can be found at www.mandatedreporterca.com.

Based on interview, the licensee did not comply with the section cited above. Licensee stated she did not have a Safe Sleep log for infants in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin to document and submit log to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed an infant asleep on a sofa which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating she understands it is unsafe for a child to sleep on a sofa and will provide a cot/mat or play yard for children sleeping to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed fire and disaster drill log which was last completed on 01/04/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit an updated fire and disaster drill log to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 3 is missing LIC995 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC995 for child 3 to CCLD by POC date.

Based on observation and interview, the licensee did not comply with the section cited above in stairs leading to second floor gate was open when LPA arrived, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Deficiency cleared during site visit, stairs leading to second floor was made inaccessible by licensee, gate was secured properly.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in hallway closet under stairwell was open with medicine, cough drops, multiple scented oils, medicine for licensee's dog was accessible under unlocked kitchen sink. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Deficiency was cleared during site visit, licensee closed door under stairwell, licensee installed lock under kitchen sink cabinet.
