Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed one scissor and two knives in sheath covers in a kitchen drawer. LPA reminded Licensee to place all sharp items in high cabinets or within child locked drawers so they are inaccessible to children. Licensee removed the sharp items and placed it in the off limit garage during today's inspection.

Resolution: Licensee to submit a written plan of correction to LPA by 02/13/2026.

Based on observation, interview and record review, the licensee did not comply with the section cited above for staff, S3 which poses a potential health, safety or personal rights risk to persons in care. Staff was observed to be missing proof of negative tuberculosis test and immunization for measles. Continued on Page 3

Resolution: Licensee to obtain proof of negative tuberculosis test and proof of immnuzation for measles for S3 and submit to LPA by 02/13/2026.

Based on observation and record review, the licensee did not comply with the section cited above for herself and 1 assistant which poses a potential health, safety or personal rights risk to persons in care. Licensee and assistant mandated reporter training expired on 09/19/2024.

Resolution: Licensee and assistant to complete the online training and submit proof to LPA by due date of 01/28/2025.

Based on observation, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.LPAs observed one infant (C1) was swaddled and placed to sleep in a play yard.

Resolution: Licensee to review safe sleep regulations and submit plan of correction due on 01/15/2025.

Based on observation, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPAs observed one infant (C1) swaddled and placed to sleep in a play yard which had loose articles including a pacifier with a clip attachment.

Resolution: Licensee to review safe sleep regulations and submit plan of correction due on 01/15/2025.

Based on observation and record review, the licensee did not comply with the section cited above for the three infants in care. Licensee stated that she observes the infants sleeping, but has not documented the 15 minute checks, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated that she will implement a 15 minute check for all sleeping infants up to age 2 and send a copy to CCL by the POC date of 04/12/2023.

Based on observations and record review, the licensee did not comply with the section cited above for Child #1 and #2 which poses a potential health, safety or personal rights risk to persons in care. Licensee stated that Child # 1 has not received his vaccinations because of
