Based on record review, the licensee did not comply with the section cited above. Four infants (C1, C6, C9 and C10) are missing sleep log which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that she will document each 15 minute check during nap time for each infant in care and will submit documentation by 10/03/25.
Based on LPA's observation, the licensee did not comply with the section cited above. Licensee is using one of the bedrooms and the kitchen which are previously identified as off limit areas for daycare purposes which pose a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that she will submit a written plan to Licensing by 10/03/25 to ensure that off limit areas will not be used for daycare until the areas are approved by Licensing and Fire Department.
