Based on record review, the licensee did not comply with the section cited above. Child 3, 4, 6 and 8 do not have LIC9224 for type A deficiency issued on 03/02/2022 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC9224 for child 1, 3 and 4 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 8 is missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC627 for child 8 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. LPA observed child 1, 3 and 4 are missing from roster of the children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee completed roster during visit. Deficiency cleared today. This is a repeat violation from report dated 03/02/2022. A $250 civil penalty is being assessed.

Based on record review, the licensee did not comply with the section cited above. LPAs did not observe a fire and disaster drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a current fire and disaster drill to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPAs observed and infant in a car seat with a blanket placed over the car seat. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's daughter removed the blanket. Licensee understands infants should not be covered with blankets while sleeping.

Based on interview, the licensee did not comply with the section cited above. Licensee's son did not have a current CPR/First Aid certificate and was left alone with children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she understands at least one adult in the home must have a current CPR/First Aid certificate when left with children.

Based on observation, the licensee did not comply with the section cited above. LPA's observed cleaning supplies under the bathroom sink, shampoos, conditioners and razors in the bathroom shower, and cleaning supplies under the kitchen sink accessible to children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed items during visit. Deficiency cited during visit.

Based on record review, the licensee did not comply with the section cited above. Child 1 and 4 are missing LIC995. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC995 for child 1 and 4 to CCLD by POC date.

Resolution: Licensee's daughter removed the child from the car seat. Licensee understands infants may not sleep in car seats and should remove car seats from day care area.

Based on observation, the licensee did not comply with the section cited above. LPA observed a 2A10BC fire extinguisher that needs to be serviced which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee must have fire extinguisher serviced or replaced and sent a photot to CCLD by POC date.
