Resolution: Licensee will provide proof of completion of the Fingerprint Clearance and Background check for one (1) Adult Assistant (S1) by the Plan of correction Due date 06/17/2025 to the department.
Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: By the Plan of Correction (POC) due date 07/16/2025, Licensee will conduct and document the fire drills and disaster drills, and send a copy of the updated drill log to the department via email.
