Based on observation and record review, the licensee did not comply with the section cited above for one assistant which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of completed Mandated Reported Training for assistant, Susan Long, to LPA by Plan of Correction date of 04/20/2023
Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Resolution: LPA provided Licensee with a copy of LIC 282- Affidavit Regarding Liability Insurance and Licensee will inform and have every parent/authorized representative sign and maintain it in child's file.
Based on record review the licensee did not comply with the section cited above in one out of one count. LPAs observed there was no LIC 9227 in any of the infant files, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Liceness will email LPA a complete LIC 9227 for all infants in care on or before 07/15/2022, to araceli.almaraz@dss.ca.gov
