Based on observation and record review, the licensee did not comply with the section cited above due to two individuals residing in the accessory dwelling unit (ADU) at the family child care home are not background check cleared which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will have the two tenants that reside in the accessory dwelling unit to complete the live scan form and visit a location to start their background check clearance and will send photo graphic evidence to the department by 11/21/2025.

Based on observation, the licensee did not comply with the section cited above due to husband smoking in the front of the home which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and LPA discussed he regulation of not smoking at the family home day care. Licensee provided a statement of her and her husbands plan that does not allow him to smoke at the facility during the inspection.

Based on record review the licensee did not comply with the section cited above in due to two assistants not having having current mandated reporter training certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of the two assistants completed mandated reporter training for child care by POC due date 12/04/2025.

Based on record review, the licensee did not comply with the section cited above in 1 out of 7 childrens files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee received the proof of immunizations for the child during the visit. This deficiency is CLEARED.

Based on record review, the licensee has not been documenting the infants naps in a 15 minute check sleep log which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will start documenting the infants naps in a 15 minute check sleep log and will submit proof to licensing by the due date 7/21/23.

Based on record review, the licensee's assistants do not have current Mandated Reporter Training for Child Care Providers which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will have her assistants complete the Mandated Reporter Training for Child Care Providers and will submit proof to licensing by 7/28/23.

Based on record review, the licensee does not have valid CPR training which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete CPR training and submit proof to licensing by the due date 8/4/23.
