Resolution: By the Plan of Correction (POC) due date, 05/19/2025, Licensee will submit the copies of Mandated Reporter Training Certificates for the Licensee and one adult assistant (S1) to the departent. Licensee will email the copies of the certificates to LPA Syeda Bahar.
Based on interview and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Licensee has not conducted and documented fire/disaster drills.
Resolution: Licensee will complete and document fire drills and submit proof to LPA by Plan of Correction date of 01/18/2024.
Based on interview and record review, the licensee did not comply with the section cited above for herself and spouse/assistant which poses a potential health, safety or personal rights risk to persons in care.

Licensee did not have proof of measles immunization and spouse/assistant did not have proof of immunizations for mealses, pertussis and influenza.
Resolution: Licensee will provide proof of immunizations for herself and assistant to LPA by plan of correction date of 01/18/2024.
Based on interview, observation and record review, the licensee did not comply with the section cited above for child, C6, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit immunization record and other required forms for child, C6, to LPA by Plan of Correction date of 01/18/2024.
