Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Last fire drill was conducted on 01/01/2024. LPA reminded licensee that fire drill need to be conducted every 6 months and documented.

Resolution: Licensee to conduct fire drill and submit proof of fire drill log to LPA by plan of correction date of 10/15/2024.

Based on observation, interview and record review, the licensee did not comply with the section cited above for herself which poses a potential health, safety or personal rights risk to persons in care. Licensee has not renewed the CPR/First Aid training.

Resolution: Licensee to complete an EMSA certified Pediatric CPR/First Aid training and submit proof to LPA by plan of correction date of 10/15/2024.

Based on observation, interview and record review the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensee did not maintain a complete current roster.

Resolution: Licensee completed the facility roster during today's inspection. Licensee to submit a plan of correction stating how to avoid this in the future by plan of correction date of 10/15/2024.

Based on observation and interview, the licensee did not comply with the section cited above for two infants in care which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit 15-minute sleep checks for two infants in care to LPA by Plan of Correction date of 09/01/2023. Licensee stated that one infant is part-time and will not be napping at the facility. Licensee will send a sleep check log for one infant in care to LPA.
