Based on LPA's observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed that the 2A10BC fire extinguisher was purchased on 12/02/20. Fire extinguisher is only good for one year from purchase date.

Resolution: Licensee states that she will purchase a new fire extinguisher and will submit proof of purchase and photo to Licensing by 02/20/26.

Based on record review, the licensee did not comply with the section cited above in 2 out of 6 children's files which poses a potential health, safety or personal rights risk to persons in care. Two children (C1 & C2) do not have sleep log.

Resolution: Licensee states she will submit completed sleep log for the two children and will submit documentation to Licensing by 02/20/26.

Per LPA’s review, Staff 1 has been working at the family day care but does not have documentation that Staff 1 is immunized against Measles, Pertussis and Influenza. This poses a potential risk to the health and safety of children in care.

Resolution: BY POC DUE DATE: Licensee will obtain documentation of the required Measles, and TB immunizations for Staff 1. A copy of the documentation will be sent to the Licensing Office as proof of correction
