Based on observation, the licensee did not comply with the section cited above. LPA observed shampoos and conditioners in children's bathroom accessible to children which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's assistant removed items during visit. Deficiency cleared today.

Based on observation, the licensee did not comply with the section cited above. LPA observed two large garbage cans in the front yard where children play filled with water. One garbage can had 23 inches of water and the other 25 inches of water which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee dumped out water during visit. Deficiency cleared today.

Based on observation, the licensee did not comply with the section cited above. Upon arrival, LPA observed licensee's son Ramon Rocha in front of the home. Licensee stated he must have stopped by to visit but left which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating she understands Ramon Rocha may not be present or live in the home until fingerprint clearance is obtained.

Based on observation, the licensee did not comply with the section cited above. The infant was on a bed surrounded by blankets and one blanket covering infant which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee removed infant during visit. Licensee will submit a statement stating she has read and understands Safe Sleep regulations.

Based on observation, the licensee did not comply with the section cited above. Licensee and assistant were not near to hear child wake up. Licensee was in the kitchen and assistant was in the day care room which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA observed an infant sleeping in an off limits bedroom with the door open about two inches which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, the licensee did not comply with the section cited above. LPA did not observe a working carbon monoxide detector. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit photos of a newly purchased carbon monoxide detector to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed left over food out, paperwork all over the kitchen table, a bug zapper full of bugs inside it and on the kitchen counter were food and drinks are located for children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit phots of a clean kitchen and kitchen table to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed two wall heaters that did not have barricades properly attached which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's son fixed the screens during the visit. Wall heaters are barricaded.

Based on observation, the licensee did not comply with the section cited above. LPA observed a baby walker and saucer in the day care room and another baby walker in the front yard were children play which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed items and placed them in an off limits living room. Licensee states she understands she may not have such items and will be donating them.

Based on observation, the licensee did not comply with the section cited above. LPA observed play yards and cribs had several blankets and the play yard did not have a fitted sheet. Licensee stated she places all the blankets in them to make it soft and comfy for the children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed blankets and placed a fitted sheet in play yard. Licensee states she understands that there shall be no blankets placed in cribs or play yards.

Based on record review, the licensee did not comply with the section cited above. Child 8 and 10 are missing LIC627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC627 for child 8 and 10 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 10 did not have current immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will sumbit current immunizations for child 10 to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. LPA did not observe a Safe Sleep log for child 10 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a statement stating she understands Safe Sleep Regulations and will begin to keep a log for all infants 24 months and under.

Based on observation, the licensee did not comply with the section cited above. LPA observed Clorox Wipes, ointments and alcohol wipes that state keep our of reach of children located near the front door accessible to children. In the front yard, LPA observed potting soil, liquid Drano and Fabuloso accessible to children. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove items and submit photos to CCLD by POC date.
