Based on record review, the licensee did not comply with the section cited above in C2 was missing updated immunization records and C3 did not have immunization records in file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of updated immunization record for C2 and immunization record for C3 to the department by due date 09/11/2025.

Based on observation the licensee did not comply with the section cited above in two cribs had mesh bumper pads which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed the mesh bumper pads during the time of inspection. Licensee and LPA reviewed the infant safe sleep regulation and licensee signed it during inspection with understanding that these should not be used in family child care homes.

Based on observation and record review, the licensee did not comply with the section cited above in C1 did not have documention of infant sleeping position, labored breathing, signs of distress, increase of body temperature which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of two weeks worth of infant sleep logs to the department by due date 09/11/25.

Based on record review, the licensee did not comply with the section cited above and assistant did not have immunization for measles which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of assitants immunization for measles, pertussis, influenza and tuberculosis test to the department by due date 09/11/2025.

Based on observation and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will check infants napping every 15 minutes and document the checks on a nap log. She will submit proof of nap log to licensing by 6/12/23.

Based on record review, the licensee did not comply with the section cited above in 1 out of 2 staff members which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated her assistant will complete Mandated reporter training and submit proof to licensing by 6/30/23.

Resolution: Licensee stated she will submit proof of Assistants immunizations (MMR, Pertussis, Influenza) to licensing by 6/12/23.
