Based on observation the licensee did not comply with the section cited above due to LPA observing one play yard that had an additional mattress on top of the mattress that was specifically made for the play yard that meet the safety standards which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed mattress at the time of inspection. Licensee understands that the mattress to be used is the one that is original to the play yard. LPA and licensee reviewed the infant sleep regulation together during todays inspection and a copy was left at the facility.

Based on observation, the licensee did not comply with the section cited above due to one child sleeping with a blanket on top of their body which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed blankets from the play yard and will let the parents know that blankets are not allowed in the play yard while infants are sleeping. LPA and licensee reviewed the infant sleep regulation together during todays inspection and a copy was left at the facility.

Based on record review, the licensee did not comply with the section cited above in S1 and S2 did not have proof of vaccinations to measles and pertussis which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee to gather the vaccination information from S1 and S2 and submit proof of vaccination to mealses and pertussis by POC due date 11/21/2025.
