Based on observation, the licensee did not comply with the section cited above one play yard had an additional mattress that was on top of the original mattress that came with the play yard which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee removed the addtional mattress from the play yard during the time of inspection. LPA and licensee reviewed the infant sleep regulation and it was left at the facility.
Based on record review, the licensee did not comply with the section cited above both assistant and licensee did not have mandated reporter training certificate on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and assistant will complete the mandated reporter training and will submit proof to the department by due date 9/12/25.
Based on record review, the licensee did not comply with the section cited above C1 and C2 were missing 15-min infant sleep logs which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete 15-min sleep log starting today 08/29/25 and will complete it for the next two weeks and submit proof to the department by due date 09/12/25 by the end of the day.
