Resolution: LIcensee agrees to have parents complete the LIC 9227's for C1 and C2 and provide proof to LPA on or before the POC due date via email or mail.
Based on record review, the licensee did not comply with the section cited above in two out of five out of children's files did not have sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee agrees to document 15-minute checks for children 0-24 months and provide proof to LPA on or before the POC due date via email or USPS.
