Based on records review, the licensee did not comply with the section cited above in assistant/adult in home did not have criminal recored clearance on file which poses an immediate health, safety or personal rights risk to persons in care. Civil penalties were assessed.
Resolution: Licensee will have criminal background clearance completed for S3 and provide proof to LPA via email. S3 will not work with daycare until background clearance is granted.

Based on records review, the licensee did not comply with the section cited above in one (S2) out of two files reviewed did not have the required immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will obtain the immunizations records for S2 and provide proof of immunization to LPA via email on or before the POC due date.
