Based on observation, the licensee did not comply with the section cited above in the backyard turf area had dog feces which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide written statement/plan to LPA stating that dog feces will be removed prior to daycare children arriving to facility and prior to children using backyard play area. LIcensee will provide signed statement to LPA via US mail on or before the POC due date
Based on observation, the licensee did not comply with the section cited above in there was no barricade or gate on stairs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will ensure that the stairs are barricaded/gated when there are children under 5 years old present. Licensee will provide picture of barricaded/gated stairs to LPA via email on or before the POC due date.
Based on observation, the licensee did not comply with the section cited above in C1 was asleep during the inspection in the baby swing until C1 awoke which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide LPA with a written statement that licensee has reviewed, understands and will comply with Title 22 Regulations regarding safe sleep. Licensee will provide statement with signature to LPA on or before the POC due date via US postal service.
Based on record review, the licensee did not comply with the section cited above in two (C1 & C6) out of the two infant records reviewed did not have sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide written statement that licensee and/or assistant(s) will keep a sleep log for children enrolled up to 24 months. In addition, licensee will provide proof of sleep logs via picture to LPA by email on or before the POC due date.
Based on observation, the licensee did not comply with the section cited above in LPA observed a sleeping infant (C6) asleep in a room with the door closed which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit to LPA a written statement that licensee has reviewed, understands and will comply with Title 22 regulations regarding safe sleep. Licensee will provide the signed statement to LPA on or before the POC due date via US mail.
Based on record review, the licensee did not comply with the section cited above in two (S1 and S5) out of five staff records did not have current mandated reporter certificates. Licensee's certification expired 02/28/2023 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee will provide proof of completion of Mandated Reporter certification for S1 and S5 via email to LPA on or before the POC due date.
Based on record review, the licensee did not comply with the section cited above in one out of five staff records did not have proof of immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of immunizations to LPA via email for S5 on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in C1 did not have LIC 9227 Individual Infant Sleep Plan on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will have parent complete LIC 9227 for C1 and provide picture to LPA via email on or before the POC due date.
Based on record review, the licensee did not comply with the section cited above in two out of two infant records (C1 & C6) did not have sleep logs which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LIcensee will provide a written statement to LPA stating that licensee has reviewed, understands and will comply with Title 22 Regulation regarding safe sleep. Licensee will also provide pictures of sleep log for all infants for period 4/15/2025 to 04/22/2025 to LPA via email on or before the POC due date.
Based on observation, the licensee did not comply with the section cited above in the licensee had 16 children present with four assistants at one time which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide a written statement and plan on how family child care home will remain in compliance with licensed capacity. Licensee will provide the written and signed plan and statement to LPA via email before close of business on POC due date.
Based on LPA Wilburn's record review, the licensee did not comply with the section cited above in 1 out of 1 persons, Licensee's First Aid/CPR certification expired on 01/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit current First Aid/CPR certification card to LPA by plan of correction date of 05/12/2023
