Based on records review, the licensee did not comply with the section cited above in facility has not conducted a disaster drill since 12/12/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof to LPA that a disaster drill has been conducted. Licensee will submit a video of the disaster drill and a copy of the disaster log via email to LPA on or before the POC due date.

Mandated reporter certification expired 06/06/2025 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete mandated reporter certification and provide proof of completion to LPA via email on or before the POC due date.
