Resolution: Licensee will obtain proof of immunizations for S3 and provide a copy to LPA via email on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in one (S3) out of three staff files did not have current mandated reporter certification on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide a copy of completed mandated reporter training for S3 to LPA via email on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in that S1 does not have a TB record on file, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide copy of TB results for S1 to LPA by 8/9/2024.

Based on record review, the licensee did not comply with the section cited above in 3 out of 5 records, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of signed LIC995A for C1, C4 and C5 to LPA by 8/9/2024.

Based on record review, the licensee did not comply with the section cited above in that the roster was incomplete: date left missing; date of birth missing; entrance date missing. C5 is not on the roster. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copy of updated roster to LPA by 8/9/2024.

Based on record review, the licensee did not comply with the section cited above in that immunization record was not on file for S1, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copy of immunization record for S1 to LPA by 8/9/2024.

Based on record review, the licensee did not comply with the section cited above in that a complete sleep log was not on file for S1, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit complete sleep log for C1 to LPA by 8/9/2024. Complete sleep log will include: Name, date, time of each 15 minute check.

Based on LPA Sanchez observation, the licensee did not comply with the section cited above in 1 out of 1 children, Child #1 did not have proof of a thorough 15 minute sleep log with all necessary information, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA Sanchez discussed the Safe Sleep regulations with licensee and advised of the required information that needs to be documented for each infant. Licensee will forward a copy of the sleep log for C1 starting today and up until the plan of correction date of 07/27/23.
