Based on record review, the licensee did not comply with the section cited above, where the LIC 9224 were not signed by the parents of enrolled children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will get the LIC 9224 signed by each parent of currently enrolled children and future enrolled children for 12 months from the date of the cited deficiency.

Based on observation, interview and record review, the licensee did not comply with the section cited above where the a child (helper's son) was left alone sleeping in the house while the staff and children were at a public park. Per Licensee's husband, he spoke with the helper who advised that she had made a unilateral decision to leave the child because he was sleeping, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The staff members arrived at the facility and the child joined the rest of the children in the day care area. Deficiency cited today will also be cleared.

Based on record review, the licensee did not comply with the section cited above where the disaster drills are not conducted or logged, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will conduct emergency drills and create a log of the disaster drills. Copy of the logs will be sent to LPA by set due date.

Based on observation, the licensee did not comply with the section cited above, where an infant child had a blanket in the crib, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The LPA advised licensee to review infant safe sleep regulations with all staff members. The child was removed from the crib where the blanket was observed during the visit. Deficiency issued today will also be cleared.

Based on observation and record review, the licensee did not comply with the section cited above in wherer a child was aboserved to be sleeping in the office, which is an off limit area, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee and LPA discussed utilizing or adding day care areas or off limits area in the facility. The child was removed from the office (off limit area) where the child was sleeping in the crib. Deficiency cited to today will be cleared.

Based on interview and record review, the licensee did not comply with the section cited above in, the staff was unable to provide 15-minute sleep logs for children under 2 years old which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The LPA advised licensee to review infant safe sleep regulations and to submit sleep logs for children under the age of 2 by set due date.
